You must have a City of Chicago Department of Finance account number to obtain an MPEA/GTT EMBLEM and/or to register for the Ground Transportation Tax.
 
Businesses needing a current City of Chicago IRIS (Integrated Revenue Information System) account number should contact the City of Chicago’s Department of Finance by sending an e-mail to revenuedatabase@cityofchicago.org with the subject line: IRIS-Need Account Number.

A false statement of material fact made on this form may violate federal, state and/or local law, and may subject any person making such statement to a range of civil and criminal penalties, such as a period of incarceration, fines and an award to the City of Chicago of up to three times any damages incurred.
