Please use this form to provide feedback about this site or to contact the Food Protection Division on any subject other than a complaint about a regulated business. Your message will be given due priority and a Food Protection representative will follow up with you if required. If you provide an e-mail address, we will send you a copy of your comments.

The City of Chicago's 311 Service Request Hotline is the preferred method for residents and visitors to register complaints. However, you may also call (312) 747-FOOD (3663) to talk with a Food Protection representative. Please have ready as much information as possible when calling to report a complaint. You will be asked to provide as much as three days of food consumption history when reporting a suspected food-borne illness. You can also email us on any subject at: food @cdph.org.
